During your appointment, NSLAC representatives will explain the Power of Attorney (POA) document(s) and assist you in signing, witnessing, and notarizing your POA, ensuring you walk out with completed POA documents.

Please note: to register, you must be a low-income resident living in North Suburban Cook County or Lake County. NSLAC will conduct a screening process at the time of registration.
